We offer our clients Web-based Process Supporting Applications for Referral Management, Utilization Review and Case Management, Discharge Planning and Real-time Reporting that gather data “in the field”, where the work is being done and by the FTEs who are already doing it.

Referral Management

Vindicet Patient Match’s web-based referral management lets you capture referral information in the field and share it instantly with review team members in multiple facilities and multiple locations. This means you can cover more referrals in less time, with fewer resources.

Using web based forms, one or more Bed Placement Specialists process patient referrals for multiple facilities/facility types.

Patient referrals are assigned to the appropriate facility based on level of care and resource availability.

Once the referral is assigned, the patient is assessed based on Clinical, Financial and Operational criteria. Assessment teams can be local to the facility, or shared across facilities.

If patient is approved for admission to a facility, patient data can be sent to your existing preadmission system. If patient is not approved, reason for denial is recorded and patient may be reassigned to another facility for consideration.

Utilization Review & Case Management

We offer a "project management" approach to utilization review and case management. When patients are admitted their status is updated in Vindicet, and baseline metrics like admitting DRG, 5/6 days, expected discharge date, medicare days and admission assessments are entered into the system.

Changes to any of these tracking metrics are entered on a daily basis where they are instantly available to the entire team.

Reporting data, including information required for CMS LTCH-CARE Data Set, can be run at any time.

Discharge Planning

Discharge planning begins during the patient's stay, with planning notes documented in the system.

At time of discharge metrics such as discharge DRG, discharge location and quality assessments are gathered and available for reporting.
